Welcome To Professional Technical Learning Centers online document reprint request form. This is a simple and painless request process that will allow us to gather enough information about your attendance to make the reprint process smoother. We will contact you if we need more information. Please be advised certificate reprint cost is $10.00. Other documents will begin at $5.00 for the first page and each additional page $0.50 per reprint. The total cost will be sent via an emailed invoice and will be required to be paid prior to the reprint.

Please keep the following in mind:
